RESUMO

A 4-month-old Domestic Shorthair cat with no clinical signs was referred for further examination of a heart murmur. An echocardiogram revealed marked right atrial dilation, extending into the left hemithorax. Computed tomography angiography was conducted to investigate further, which revealed a balloon-shaped, contrast-filled cavity on the cranial and left side of the chest that connected to the right atrium through a narrow passage. This was diagnosed as a congenital right atrial diverticulum. Treatment included clopidogrel to reduce the risk of thrombus formation. Two months after the initial diagnosis, castration surgery was successfully performed without complications. The cat died suddenly at home 10 months after diagnosis. To the authors' knowledge, this is the first right atrial diverticulum reported in a cat.

INTRODUCTION: English springer spaniels have larger, rounder ventricles than most other breeds. How this geometry impacts responses to volume overload remains unknown. We compared left ventricular size between English springer spaniels and two similarly sized sporting breeds (Border collies and Labrador retrievers) in naturally occurring chronic left ventricular volume loading conditions (mitral regurgitation and patent ductus arteriosus [PDA]) to examine whether differences in remodelling responses exist between these breeds. ANIMALS, MATERIALS AND METHODS: We searched records for cases of mitral regurgitation and PDA in three breeds. We recorded age, sex, presence of congestive heart failure (CHF), body weight and specific echocardiographic variables. We compared normalised measures of left ventricular size between breeds. Cases with CHF were further examined as a separate group. RESULTS: One-hundred-and-ninety-one dogs were included: 110 with degenerative mitral valve disease, 42 with mitral dysplasia and 39 with PDA. One third of all cases had CHF. All measures of left ventricular size were larger in English springer spaniels in mitral regurgitation cases (P<0.001), whereas PDA cases did not differ. English springer spaniels with PDA resulting in CHF had larger systolic dimensions and volumes than similarly affected non-English Springer Spaniel dogs (P=0.003). CONCLUSIONS: English springer spaniels have greater left ventricular dimensions when exposed to chronic mitral regurgitation, compared with Border collies and Labrador retrievers, but not when exposed to volume overload from a PDA. English springer spaniels differ in their left ventricular morphology from two other sporting breeds, supporting previous studies that they have a unique cardiac morphotype.

OBJECTIVES: The objectives of this study were to determine whether conducting a clinical audit was achievable in a group of centres that perform interventional cardiac procedures and to report the success and complications rates in dogs diagnosed with patent ductus arteriosus. METHODS: This was a multicentre, European-wide, prospective study. Patient data were entered into a bespoke database prior to commencing interventional closure of patent ductus arteriosus in all animals undergoing this procedure during the study period. The database was designed to gather clinical audit information, after completion of the procedure, such as discharge outcome, complication rate, and medium-term outcome. RESULTS: A total of 339 cases were included from five participating centres. The process of performing clinical audit was achieved in all centres. Successful discharge outcome was 95.9% with a complication rate of 4.1%. The procedure-related mortality was 0.6%. 149 cases (43.9%) were either lost to follow-up or had not yet had a follow-up within the time period. Of the remaining 169 cases in which follow-up was available, 157 (92.9%) cases had a successful medium-term outcome CONCLUSIONS: This study demonstrates that the process of performing a clinical audit is achievable in veterinary clinical interventions across different centres. These results provide a benchmark for future comparison in our ongoing clinical audit and validate the process of clinical audit for other centres performing cardiac interventions. The use of clinical audit should be considered in other aspects of veterinary medicine.

An 18-month-old Springer Spaniel was presented for investigation of ascites, exercise intolerance and suspected cor triatriatum dexter. Division of the right atrium into two separate chambers by an anomalous perforated membrane consistent with cor triatriatum dexter was confirmed during echocardiography. However, a routine agitated saline contrast (bubble) study yielded results that could not be explained by the congenital heart defect alone. Computed tomography angiography was performed and revealed a dilated ventral internal vertebral venous plexus and a short, dilated vessel in the midlumbar region redirecting blood flow away from the caudal vena cava and into the azygos vein. Balloon dilatation of the orifice within the cor triatriatum dexter membrane successfully reduced pressure within the caudal chamber of the right atrium and enhanced return via the caudal vena cava with subsequent resolution of clinical signs. This case report highlights the use of cross-sectional imaging in dogs with cor triatriatum dexter and unexpected contrast study results, as a means by which concurrent vascular anomalies may be identified.

RESUMO

Cough has been historically reported as a major clinical sign of cardiogenic pulmonary oedema in dogs. However, recent evidence appears to contradict the traditional dogmatic approach that linked cough to congestive heart failure in dogs. Here we use a question-based format to introduce and discuss the modern evidence regarding "cardiac cough" and the interpretation of this important but often misleading clinical sign.

OBJECTIVES: Assess global circumferential and radial systolic and diastolic myocardial function with speckle tracking echocardiography (STE) in healthy Great Danes (GD) and in GD diagnosed with dilated cardiomyopathy (DCM). ANIMALS: Eighty-nine GD were included in the study: 39 healthy (normal group [NORMg]) and 50 diagnosed with DCM (DCMg). METHODS: This was a retrospective study. Signalment and echocardiographic diagnosis were obtained from the medical records of GD assessed between 2008 and 2012. Speckle tracking echocardiography analysis of circumferential (C) and radial (R) strain (St) and strain rate (SR) in systole (S), early (E) and late (A) diastole was performed at the levels of the mitral valve (MV), papillary muscles (PM) and apex (Ap) of the left ventricle. Univariable and multivariable analysis was performed to identify differences between groups. RESULTS: Speckle tracking echocardiography variables increase from the MV towards the Ap of the left ventricle in both NORMg and DCMg dogs, some reaching statistical significance. Most of the variables (28/31) were lower in DCMg than in NORMg dogs: statistically significant variables included radial SR at the Ap in systole (p=0.029), radial strain at the PM (p=0.012), circumferential SR at the PM in systole (p=0.031), circumferential and radial SR at the MV in early diastole (p=0.019 and p=0.049, respectively). CONCLUSIONS: There are significant differences in STE variables between NORMg and DCMg Great Danes, although the overlap between the two groups may indicate that these variables are not sufficiently discriminatory. STE variables are not sufficiently sensitive to use in isolation as a screening method.

OBJECTIVES: To describe left atrial size, left atrial volume, left atrial function and left ventricular diastolic function in healthy cats and those with hypertrophic cardiomyopathy without and with congestive heart failure. METHODS: A retrospective study of 61 client-owned, 21 healthy, 21 asymptomatic hypertrophic cardiomyopathy and 19 with hypertrophic cardiomyopathy and congestive heart failure cats. Data were retrieved from clinical records and echocardiography archives. Left atrial diameter and volumes were measured. Left atrial function was investigated using changes in diameter (fractional shortening) and volume (Simpson's method; left atrial ejection fraction). Conventional echocardiographic indices of left ventricular diastolic function were recorded. RESULTS: Left atrial diameter and left atrial volume measurements were significantly higher in hypertrophic cardiomyopathy with congestive heart failure cats compared with asymptomatic hypertrophic cardiomyopathy and healthy cats (P < 0·001). Left atrial passive, active and complete ejection fraction distinguished between hypertrophic cardiomyopathy with congestive heart failure and asymptomatic hypertrophic cardiomyopathy (P < 0·001). Hypertrophic cardiomyopathy with congestive heart failure cats had significantly lower mitral A wave velocity (P = 0·016) and atrial complete emptying based on diameter and volume measurements (P = 0·008 and P < 0·001, respectively) compared with asymptomatic hypertrophic cardiomyopathy cats. CLINICAL SIGNIFICANCE: Left atrial volume is obtainable by echocardiography in cats. Left atrial volume and atrial function may indicate chronicity and severity of diastolic dysfunction associated with hypertrophic cardiomyopathy and congestive heart failure. Left atrial function was reduced in cats with hypertrophic cardiomyopathy and congestive heart failure compared with healthy and asymptomatic hypertrophic cardiomyopathy groups.

BACKGROUND: Boxer dogs are reported to be predisposed to arrhythmogenic right ventricular cardiomyopathy (ARVC), but the natural history has not been well characterized and inconsistent diagnostic criteria have been applied to identify affected dogs. Echocardiographic examination findings are unremarkable in many affected Boxer dogs, and in these dogs, 24-hour ambulatory ECG (Holter) monitoring often is used for diagnostic and prognostic purposes, despite limited information available relating Holter findings to outcome. HYPOTHESIS/OBJECTIVES: Boxers with complex ventricular arrhythmias at initial presentation will have shorter survival times. The objective was to investigate the prognostic value of Holter monitoring in Boxer dogs. ANIMALS: One hundred and twenty-two Boxer dogs seen at 3 university referral hospitals. METHODS: Retrospective study. Survival times were obtained for Boxer dogs evaluated by echocardiography and a 24-hour Holter ECG. Kaplan-Meier survival analysis was used to estimate the median survival time and Cox proportional hazards analysis was used to identify variables independently associated with cardiac mortality. RESULTS: Outcome data were obtained for 122/163 dogs meeting the inclusion criteria. Of the 70 dogs that had died, 45 were considered to have suffered cardiac-related deaths. Median survival was significantly longer in dogs with a left ventricular systolic diameter (LVIDs) ≤ 35 mm compared with those with LVIDs > 35 mm (P < .001). Multivariable analysis in dogs with LVIDs ≤ 35 mm showed that the presence of ventricular tachycardia, age >4.5 years, and male sex were independent predictors of cardiac mortality. CONCLUSIONS AND CLINICAL IMPORTANCE: Holter monitoring in Boxer dogs provides valuable prognostic information.

We assessed the costs and effectiveness of rapid HIV testing services provided to transgender communities in New York City and San Francisco from April 2005 to December 2006. Program costs were estimated based on service provider's perspective and included the costs attributable to staff time, incentives, transportation, test kits, office space, equipment, supplies, and utilities. The average annual numbers of persons tested were 195 and 106 persons and numbers notified of new HIV diagnoses were 35 (18.2%) in New York City and 8 (7.3%) in San Francisco, respectively. The estimated annual program costs were $125,879 and $64,323 and average costs per person notified of new diagnosis were $3,563 and $8,284 in New York City and San Francisco, respectively. The primary reason for differences in program costs by site was differences in the proportion of undiagnosed HIV infection among persons tested. Our findings can inform decisions about program planning and allocation of limited HIV testing resources.

OBJECTIVES: This article describes the demographic and behavioral characteristics, human immunodeficiency virus (HIV) testing history, and results of HIV testing of transgender (TG) people recruited for rapid HIV testing by community-based organizations (CBOs) in three cities. METHODS: CBOs in Miami Beach, Florida, New York City, and San Francisco offered TG people rapid HIV testing and prevention services, and conducted a brief survey. Participants were recruited in outreach settings using various strategies. The survey collected information on demographic characteristics, HIV risk behaviors, and HIV testing history. RESULTS: Among 559 male-to-female (MTF) TG participants, 12% were newly diagnosed with HIV infection. None of the 42 female-to-male participants were newly diagnosed with HIV. A large proportion of MTF TG participants reported high-risk behaviors in the past year, including 37% who reported unprotected receptive anal intercourse and 44% who reported commercial sex work. Several factors were independently associated with increased likelihood of being newly diagnosed with HIV infection among MTF TG participants, including having a partner of unknown HIV status in the past year; being 20-29 or > or = 40 years of age; having last been tested for HIV more than 12 months ago; and having been recruited at the New York City site. CONCLUSIONS: Based on the high proportion of undiagnosed HIV infection among those tested, TG people represent an important community for enhanced HIV testing and prevention efforts. MTF TG people should be encouraged to have an HIV test at least annually or more often if indicated, based upon clinical findings or risk behaviors. Efforts should continue for developing novel strategies to overcome barriers and provide HIV testing and prevention services to TG people.
